Durham Green has been approved by the Department of Health to provide Community Aged Care Packages (CACPs) in South West Sydney. That means that if a resident at Durham Green has more complex care needs but would still like to remain living in their own home, they will be provided with a planned package of care.
Funded by the Australian Government, the types of services that may be provided include:
- Personal care
- Social support
- Transport to appointments
- Home help
- Meal preparation
To receive a CACP, you must be assessed by an Aged Care Assessment Team. For more information call the Aged and Community Care Information Line on 1800 500 853 or Commonwealth Carelink Centres on 1800 052 222.
