Blue Hills Community Care Package

We are proud of the fact that we have recently been approved to provide Community Aged Care Packages (CACPs) in South West Sydney by the Department of Health (call 1300 551 338 to see if CACPs are right for you or a family member).

That means that, if a resident at Durham Green has more complex care needs but would still like to remain living in their own home, they will be provided with a planned package of care. Funded by the Australian Government, the types of services that may be provided include:

  • personal care
  • social support
  • transport to appointments
  • home help
  • meal preparation
  • gardening

To receive a CACP, you must be assessed by an Aged Care Assessment Team. For more information call the Aged and Community Care Information Line on 1800 500 853 or Commonwealth Carelink Centres on 1800 052 222.
